At Apria Healthcare, we've created a benefits package that meets your real-world needs, fosters a healthy lifestyle and gives you plenty of options.
You will be eligible for benefits if you are a full-time employee who is scheduled to work at least 35 hours a week or a part-time employee scheduled to work at least 24 hours a week. Some benefits, such as our 401(k) Savings Plan, are immediately available.

MEDICAL PLANS
  • Anthem PPO/Aetna POS Plan (Full-time Only)
    Reimburses 80% of eligible in-network expenses (or 50% when services are not received from an in-network provider) after deductible.
  • Anthem /Aetna EPO Plan (Full-time or Part-time)
    Reimburses 80% of eligible in-network expenses (no out-of-network coverage). No annual deductible.
  • Anthem/Aetna High Deductible Health Plan
    Allows to you establish a tax-free, interest-earning account to cover any out-of-pocket expenses.
All plans include prescription drug coverage through Medco.


DENTAL PLANS
  • MetLife Standard Dental Plan
    This individual deductible plan reimburses 100% of eligible preventive services and fillings, 80% of eligible basic services and 50% of eligible major services.
  • MetLife Basic Dental Plan
    This lower option plan reimburses 90% of eligible preventive services and fillings, 70% of eligible basic services and 40% of eligible major services when services are received from an in-network provider.

VISION PLAN
Provides comprehensive coverage through either VSP or private provider.


EMPLOYEE ASSISTANCE PROGRAM (EAP)
Horizon Health provides you and your family members with confidential guidance for a variety of personal and work-related issues that may affect your daily life. Includes a 24-hour, toll-free help line.
